General Overview: The Community Linkages Facilitator will play a crucial role in strengthening the linkage between community-based structures and health facilities to ensure effective referrals and HIV cascade management. This position aims to improve health service uptake and achieve UNAIDS goals by working closely with OVCs, youth, and caregivers.
Key Responsibilities:
- Collaborate with District-Based Teams and Para-Social Workers (PSWs) to track newly tested positive individuals and link them to care.
- Facilitate monthly meetings to review the performance of service uptake and referrals.
- Provide technical assistance on referral processes and integrated support within the community.
- Ensure clients are reminded of their clinic appointments and guide them through the health care system.
- Participate in health education and patient tracking activities.
- Follow up with clients to ensure retention in care and provide additional support as needed.
- Document referrals and maintain accurate records.
- Compile and share reports on beneficiaries and their service uptake.
Academic Qualification:
- Minimum of Ordinary Level Certificate or its equivalent.
- Training in Social Work, Community Development, Public Health, Sociology, Project Planning, Community Health, or Child Protection is advantageous.
Person Specification:
- At least 3 years’ experience in community work within a government institution, health facility, NGO, or USAID-funded program.
- Skills in service delivery, referrals, linkages, and OVC activities.
- Excellent communication and interpersonal skills.
- Fluent in local languages and English.
- Willingness to travel and work in a multi-disciplinary team.
- High personal and professional integrity.
- Computer literacy is essential.
